ASSUMPTION OF RISK AND RELEASE OF LIABILITY

Customer agrees to have scrap materials unloaded from their vehicles by Yard Attendant/employees, machinery, or equipment.

Customer acknowledges that during the process of unloading material, there is a potential risk of damage to their vehicle, equipment, or trailer, which may include but is not limited to:

• Scratches, dents, or other physical damage to the exterior of the vehicle
• Damage to the vehicle's interior
• Damage caused by manual labor, machinery, or equipment

The Customer voluntarily accepts these risks and understands that they are inherent in the nature of the activity.

In consideration for being permitted to unload scrap materials at the facility, the Customer agrees to release, discharge, and hold harmless Lewis Salvage Shred Services, its owners, employees, agents, and contractors from all claims, demands, actions, or causes of action for damages, including vehicle/equipment/trailer damage, arising out of or relating to the unloading process.

This release includes, but is not limited to, any damage to the Customer's vehicle resulting from negligence, accidents, or any other cause during the unloading of scrap materials.

The Customer agrees that they are solely responsible for the safety and condition of their vehicle during the unloading process. The Customer understands that they may choose to unload the vehicle themselves or request assistance, but Lewis Salvage Shred Services is not liable for any resulting damage, regardless of how the unloading is conducted.

By signing this form, the Releasor acknowledges that they have read and understood the terms of this Agreement and are voluntarily signing it without any duress or undue influence.
